As a CNA or tech, you might have eight, ten, or more residents to keep up with — vitals due, people to turn, weights, blood sugars, and a constant stream of call lights. A nurse’s brain sheet doesn’t fit the rhythm. You need something that tracks the hands-on basics across a lot of people, fast.

This sheet keeps it simple and scannable, with a row per resident and columns for the things you’re responsible for through the shift.

What’s on the CNA report sheet

  • Resident & room: name, room, and any quick alerts (fall risk, diet, mobility).
  • Vitals: a spot to jot each set as you take them.
  • I&Os: intake, output, and toileting tracked through the day.
  • Turns & ADLs: q2 turn checkboxes, bath, and activity so you can prove the care got done.
  • Notes: anything to pass along to the nurse — skin, appetite, a change you noticed.
You’re the early warning system. The “notes” column matters — the skin change or the resident who “just isn’t themselves” that you flag is often what catches a problem early. Write it down and tell the nurse.
